What You Bring To Cambia

The Care Advocate I participates in the integrated approach to care management for members. This role provides outreach as part of specific care gap initiatives or as an on-going part of a care management team. The position works with members to achieve their personal health outcomes by removing barriers and providing support and education.

Qualifications
• High school diploma
• Medical certification - Certified Nursing Assistant (CNA) or Medical Assistant preferred
• 3 years of experience in health plan Customer Service, Clinical Intake, Provider office, or an equivalent combination of education and experience

Skills And Attributes
• Effectively interact with a wide variety of individuals on highly sensitive topics, providing excellent customer service through listening, patience, empathy, and maintaining confidentiality.
• Possess general organization, data entry, and administrative skills, with the ability to effectively handle multiple tasks and prioritize workload.
• Proficient in general computer skills, including Microsoft Office, Outlook, Internet search, and basic knowledge of medical terminology.
• Able to work independently, prioritize work, meet deadlines, and achieve operational standards, with the ability to research and problem-solve in a creative manner to reach a solution.
• Willing to work a varied schedule, including early evenings, and able to identify when it is appropriate to refer to others, maintaining confidentiality and professionalism in all interactions.

What You Will Do At Cambia
• Initiates outbound telephone calls or emails to members identified for the Regence Care Management Programs for defined care gap initiatives.
• Utilizes motivational interviewing and behavioral change principles to encourage engagement in health-related activities and programs.
• Educates members by providing the tools and information they need to adhere to nationally recognized preventive screenings, discharge and treatment plans.
• Works with a Care Manager to assure that member needs are met through identification and referral to appropriate resources.
• Makes referrals to licensed clinical staff as appropriate.
• Utilizes the applicable care management platform to appropriately record and track member interactions.

The starting hourly wage for the Care Advocate role is $24.00-$30.00/hour, depending on candidate's geographic location, certification and experience. Full pay range for the Care Advocate role is $24.00-$41.50 an hour

The annual incentive payment target for this position is 5%.
