Bilingual Remote Patient Monitoring Nurse (Contract)

Key Responsibilities • Monitor and assess patient-generated health data blood pressure, weight, glucose, pulse oximetry) through the Vive Now portal. • Identify abnormal readings or concerning trends and perform timely outreach to patients per clinical protocol. • Provide patient education on chronic conditions, device use, and lifestyle modifications to support care plan adherence. • Document all interactions, assessments, and interventions accurately in the electronic health record (EHR). • Collaborate with providers, care managers, and other clinical team members to address patient needs and escalate concerns appropriately. • Assist with onboarding new patients, including device education and troubleshooting. • Maintain strict adherence to HIPAA and all organizational privacy and security standards. • Promote patient engagement, satisfaction, and health improvement through consistent, supportive communication. Qualifications • Active, unrestricted LPN License in the state of practice; multistate licensure preferred. • At least 1 year of clinical nursing experience; experience in telehealth, triage, case management, or chronic disease management preferred. • Bilingual Vietnamese level. • Strong understanding of chronic conditions such as hypertension, diabetes, CHF, COPD, and obesity. • Excellent verbal communication, patient education, and motivational interviewing skills. • Ability to work independently, manage time effectively, and prioritize patient needs. • Strong critical thinking and clinical judgment skills. • Proficiency with EHR systems and comfortable navigating technology. Compensation • Part-time and full-time position (contract only) • Part-time: 10+ hours per week (8 to 5 pm EST) • $25 per hour.
