Responsibilities Assessing, planning, implementing, and coordinating all case management activities with members to evaluate the medical needs of the member to facilitate the member's overall wellness Effectively manage a caseload that includes supportive and medically complex members Develop a proactive course of action to address issues presented to enhance the short and long-term outcomes as well as opportunities to enhance a member's overall wellness through integration Determine appropriate services and supports due to member's health needs; including but not limited to: Prior Authorizations, Coordination with PCP and skilled providers, Condition management information, Medication review, Community resources and supports Skills Bi-lingual (English and Spanish) Must reside in zip code 33615 or 33635, or within 2-3 miles Willing and able to travel 50-75% of their time to meet members face to face in Hillsborough County and surrounding areas in FL 1+ years of experience in behavioral health or a long-term care setting Computer proficiency in bolthires Word, Excel, and Outlook Bachelor's Degree required, in Social Service or related field (social services field - psychology, health administration) Ability to multitask, prioritize and effectively adapt to a fast-paced changing environment Effective communication skills, both verbal and written Managed care experience Case management and discharge planning experience Benefits Affordable medical plan options, a 401(k) plan (including matching company contributions), and an employee stock purchase plan.No-bolthires programs for all colleagues including wellness screenings, tobacco cessation and weight management programs, confidential counseling and financial coaching.
