Behavioural Health Professional/ Social Worker/ Mental Health Counsellor
Job Title: Care Manager Transition of Care
Duration: 03 Months Contract
Desired Start Date/ End Date: 9/22/2025 - 12/31/2025
Location: Remote β NE (Columbus Area Preferred)
Shift Type: Thurs-Mon (Flexible between hours of 2pm-8pm), OFF on Tues-Wed (Flexible between hours of 2p-8p), Must work evenings and weekends (Flexible with weekend hours, as well, but MUST work 8 hours (between 8a-8p)
Job Duties:
β’ Looking for member outreach from 3-8 pm 5 days a week and 8 hour weekend days.
β’ Open to flexibility of time on weekends depending on best times to reach members and facilitate follow up.
β’ Staff would coordinate with local CM TOCs virtually. Member outreach will be via phone.
Experience required (Must Haves)
Strong Communication Skills
Knowledge of available behavioral health resources
Ability to manage multiple technology systems
Must haves
Empathy, strong communication skills, Critical thinking and ability to problem solve, ability to make decisions independently, self-starter & self-disciplined.
Disqualifiers:
Donβt have a NE License or RN not in a compact state.
Performance indicators:
We look at number of completed assessments to monitor performance
Education/Certification
Masters Degree in Behavioral Health Field or Registered Nurse with extensive Psych experience
Licensure:
LMHP, LCSW, LISW, LMSW, LMFT, LMHC, LPC, or an RN with extensive Psych experience
