We are seeking a talented Backend Engineer to join a healthcare-focused data platform team, building scalable, high-performing services for clinical and financial data integrations. In this role, you will work with FHIR-based workflows, distributed systems, and cloud architectures to enable secure and compliant data exchange. You will collaborate with cross-functional teams to design robust ingestion pipelines, optimize system performance, and ensure adherence to healthcare compliance standards. This position offers a hands-on opportunity to impact real-world healthcare solutions affecting millions of patients. Remote work across Latin America is supported, with alignment to the US-based team’s schedules. Accountabilities Design and implement scalable ingestion pipelines for FHIR-based clinical and administrative data. Architect distributed backend systems using Java, Spark, Kafka, and Kubernetes. Develop APIs and services to validate, transform, and enrich healthcare data. Ensure privacy, security (HIPAA), and full data lineage throughout the integration stack. Optimize batch and real-time data flows for latency, throughput, and cost efficiency. Collaborate with product, DevOps, and architecture teams to deliver critical platform features. Stay current with FHIR (R4, R5), US Core, and TEFCA standards, guiding internal adoption and best practices. Requirements 5+ years of professional software engineering experience. 2+ years of experience with healthcare data integrations, particularly FHIR. Expertise in Java, JVM optimization, and distributed frameworks such as Spark or Flink. Hands-on experience with FHIR resources, bulk-data, SMART on FHIR, and HL7 v2/CDA. Experience with Kafka, REST APIs, and event-driven architectures. Familiarity with cloud services, preferably AWS (Lambda, EKS, S3, RDS, etc.). Understanding of HIPAA, data security, and healthcare compliance. Strong problem-solving skills and ability to explain technical solutions to technical and non-technical stakeholders. Nice to have: Experience with backend languages like Python or Go. Familiarity with FHIR Bulk API, Kafka Streams, or Kafka Connect. Exposure to Terraform, GitHub Actions, Argo Workflows, or Helm.
