Jefferson Street is a Group Home for Mental and Behavoiral Health with 8 residents, 4 males and 4 females, is looking for Wellness Coaches to assist our 8 independent residents in their day to day living activies and reach their Person Centered Care Treatment Plan goals. Assisting with ADLs, taking residents on community outings and medical appointments. The mission of Resources for Human Development (RHD) is to provide caring, effective and innovative services that empower people of all abilities as they build better lives for themselves, their families and their communities. RHD empowers individuals to develop skills for daily life, Support the personal growth of the people we serve, Encourage independence through personalized plans of care, and Plan events and outings to build independence and connections within the community. Located 2 minutes from Christiana Wilmington Campus Hospital. Bachelor Degree is required for this position. $19.29 hourly. Avaiable shifts: Full Time- Evening Shift: Monday-Friday: 3:00P to 11:00P Part Time- Saturday and Sunday: 7am-3pm, and 3pm-11pm Responsibilities/Duties Provide supervision of individuals with focus on person centered care; supporting individuals to work towards their goals and other person centered support in their treatmentplans. Complete required daily documentation and other assigned documentation in Electronic Health Records (Avatar). Become LLAM certified and administer medications to the residents. Provide transportation to residents for medical appointments and community outings. Attend monthly staff meetings remotely. Facilitate groups with residents. Complete assigned monthly trainings (online via Cornerstone). Maintain annual TB/PPD, LLAM Training (medication administration), and First Aid CPR certification. Other duties as Assigned. Required Experience Prior experience in social sevices, or human services preferred. Experience working in a group home setting preferred, but not required. Required Education Bachelors Degree in social sevices, human services, psychology, behavioral health or related degree preferred and experience in mental health. Required Qualifications Valid Drivers license Prior experience in Mental and Behavioral Health Group Home (Preferred) Program Summary Great opportunity to work in Behavior/Mental Health with focus on providing centered/individualized medical and mental healthcare to our eight residents (4 male and 4 female) and help the residents thrive and become more independent and be active praticipants in the community.
