Responsibilities:
What You Will Do: · All tasks as directed by RN Case Managers, Nurse Manager, or Area Clinical Leader.· Care coordination with physicians and other healthcare entities. This includes, but is not limited to:Patient referralsMD office visit notesH&PSharing of Plan of Care with other entitiesTracking of supplemental physician orders and plans of careInput of basic non-clinical information into the EMRPerforms other related duties as assigned
Qualifications:
What You Will Bring:Ability to exercise independent judgmentAbility to demonstrate above accuracy attention to detail to ensure accuracyAbility to read and interpret medical record data for chart auditEducation and Experience:High school diploma or GED requiredCMA/QMA preferredOne (1) year experience in care coordination preferredKnowledge of service regulations and processPhone/computer experience The above statements are intended to describe the general nature and level of work being performed by people assigned to this classification.
